Trenton Health Team Announces Funding Opportunity to Support Implementation of Community Health Projects As Part of the Trenton Health Team Accountable Care Organization

Trenton (February 5, 2019) – Trenton Health Team Medicaid Accountable Care Organization (THT-ACO) is pleased to announce a funding opportunity to support implementation of Community Health Improvement Projects in the greater Trenton area.

THT-ACO has set aside up to $200,000 to support eligible projects by community-based organizations in the greater Trenton area (specifically, ZIP codes 08608, 08609, 08611, 08618, 08629, and 08638). Support between $1,000 and $25,000 will be awarded for evidence-based or novel projects serving Medicaid beneficiaries and addressing one of the following:

  • Chronic disease prevention or management (e.g., diabetes, hypertension, CAD, cancer)
  • Maternal and child health
  • Substance use and behavioral health
  • Social determinants of health (e.g., transportation, housing, food access, safety)
  • Clinical-community linkages
  • Access to primary and/or specialty care

The Medicaid ACO demonstration project provides the New Jersey Medicaid program an opportunity to explore innovative system redesign, including evaluating how care management, care coordination, and other community-based services could best be delivered to high risk, high cost utilizers, and stretching the role of Medicaid beyond just medical services to integrate social services.

Applications must be received no later than Thursday, February 28, 2019 at 4:30 PM.
