The Community Report documents Trenton Health Team’s early progress in its quest not just to change the lives of Trenton residents but also to communicate our successes and challenges in order to help other cities.
Formally incorporated in 2010, THT focused on helping translate the tenets of healthcare reform into scalable practices that give vulnerable populations a new chance to lead healthy lives within a holistic, sustainable system.
THT—the first transformative collaboration of hospitals, physicians, nurses, and other healthcare providers and social service agencies ever in New Jersey’s capital city—introduced innovative approaches to healthcare where they are desperately needed.Â
By forging partnerships, creating new educational pathways, and engaging city residents in creative ways, THT transformed healthcare for city residents with a goal is to expand access to high-quality, coordinated care and make Trenton the healthiest city in the state
