Since its founding in 2006, Trenton Health Team (THT) has spear-headed many programs and initiatives to promote health and well-being for Trenton residents. Listed here are THT programs that were completed or are now led by partner organizations:
Early efforts to improve access to primary care, improve outcomes, and lower costs included the introduction of Open-Access or Advanced Access Scheduling to Trenton’s outpatient clinics, Inroads for Health: Taking It to the Streets, Faith in Prevention programming, and more.
With generous funding from the Robert Wood Johnson Foundation and Janssen, Trenton Health Team and the City of Trenton Department of Health & Human Services took the Wheels to Wellness mobile health van to organizations and events across the community. The Wheels to Wellness mobile health van is now operated by Henry J. Austin Health Center.
THT, in partnership with the National Council for Behavioral Health, completed a two-year Foundations for a Trauma-informed Culture of Health initiative, with funding from the Robert Wood Johnson Foundation.
