Trenton Health Team Receives $25,000 from The Horizon Foundation for New Jersey to Connect Trenton Residents with Essential Health Services
Trenton (January 28, 2019) – Trenton Health Team (THT) has received a grant in the amount of $25,000 from The Horizon Foundation for New Jersey to help fund the Trenton Health Team Peer Health Program, which empowers local residents to connect their neighbors to essential health and social services through the NowPow referral system along with diabetes education and self-management support.
The primary goal of the project is to improve the health and well-being of Trenton residents through peer support and health education. The project will establish two different peer groups, one of which will be faith-based. It will include recruiting a peer mentor from within each group, who will be hired by THT. Peer mentors will be trained to utilize the NowPow system to make connections with needed resources in the areas of food access, nutrition education, housing, utility assistance, substance abuse and behavioral health, smoking cessation programming, physical activity, and employment and job training services.
The NowPow system, one of many resources provided to local leaders through the Trenton Health Team Peer Health Program, is a social services technology platform that connects people to community resources that help address their health conditions and social needs by making personalized referrals. Using the system, staff, social workers, case managers, and THT peer health mentors, can screen a patient for social determinants of health and generate a curated list of resources based on the patient’s geography and unique needs.
