Trenton Health Team is committed to ensuring everyone has access to health care while also addressing societal factors such as food, housing, jobs, and safety.
In our Community Care model, our team made up of Community Health Workers, educators, patient navigators and registered nurses with lived experience in the Trenton community, provide one-to-one support and guidance to Trenton residents to help them navigate their own health and wellbeing challenges and advocate for themselves.
