Mission & Vision

Mission

Trenton Health Team’s mission is to improve health and well-being in the community of greater Trenton, New Jersey.

At Trenton Health Team (THT), we believe health and well-being is fundamental to a healthy society. 

Our team is committed to ensuring everyone has access to health care while also addressing societal factors such as food, housing, jobs, and safety.

To improve the health of the greater Trenton community, our team works in three unique and interdependent ways:

  • Community Care: THT employs, supports and deploys people with lived experience in the Trenton community to work with individuals and community groups, assessing health care needs and helping residents learn how to care for – and advocate for – themselves.
  • Community Partnerships: THT lends capacity to the web of organizations working on community health, through its work as a convenor, program manager, lead applicant for funding, and fiscal sponsor. 
  • Data & Research: THT harnesses community experience combined with rigorous data analysis to help our partners to pinpoint gaps and barriers to service, and identify ways to improve service delivery and health systems.

Vision

THT’s vision for a “healthy Trenton” incorporates social determinants of health, including:

Partnership

THT works with dozens of partners to identify gaps in access to health and well-being, and then addresses those gaps through collaborative projects and by helping organizations access the data they need to design effective programs and policies. Collaboration locally, and across the state and country, is a key component of THT’s work.

History

THT has been serving Trenton since 2006, a unique response to a healthcare crisis spurred by the closure of Mercer Medical Center, and the product of the collaboration of competing partners and the city government. What began as an unlikely partnership among competitors has grown into a small but mighty organization consisting of about 40 staff members who are deeply rooted in the community we call home. We respond to the ever-changing and complex needs of Trenton by building unique partnerships, piloting new projects and ideas, and creating novel tools that help us and our partners improve health outcomes across the city. In a city that is too often overlooked, we’re proud to be the neighbor you can count on to build trusted relationships and a healthier, more just Trenton.

NJ Regional Health Hubs

THT is one of New Jersey’s first Regional Health Hubs created in 2020 through legislation signed by Governor Phil Murphy. Regional Health Hubs provide a community-based structure for convening stakeholders, planning and providing health interventions in order to eliminate healthcare fragmentation hampering care. As an RHH, THT can better serve residents receiving healthcare coverage through Medicaid — those who often lack access to the resources needed to be, and stay, healthy. New Jersey’s regional partnerships (THT, Camden Coalition of Healthcare Providers, Healthy Greater Newark, and Health Coalition of Passaic County) work with both clinical and social service providers to explore innovative structures and practices to integrate, coordinate and align disconnected programs in order to make communities healthy.
