Wheels to Wellness
NOTE: The Wheels to Wellness program is no longer facilitated by Trenton Health Team. As of January 1, 2018, the program will be facilitated by Henry J. Austin Health Center. For more information, call 609-278-5900.
With generous funding from the Robert Wood Johnson Foundation and Janssen, Trenton Health Team and the City of Trenton Department of Health & Human Services were able to expand on-site health services by taking the Wheels to Wellness mobile health van to organizations and events across the community, providing free health screenings, referrals, and educational information to many of the city’s most vulnerable residents.
Services offered around the City
Clinicians, clinical students and community health workers conducted blood pressure, body mass index and blood glucose screenings, provided health education and offered general and specialized health assessments that were obtained through an examination and questionnaires. Mobile health clinic staff also provided referrals to resources and health services that were not provided on the van.
Advancing Inroads to Health
Our project, Advancing Inroads for Health, which aimed to evolve this work into a cohesive and deliberate community-wide program, established more formal procedures, goals, and branding/identity, operating on a year-round basis.
At the program’s outset in 2016, our immediate goal was to fill the gap in access to healthcare by providing comprehensive care to the residents of Trenton, irrespective of immigration or health insurance status. Our long-term goal was to address the disparities that exist within the community regarding health literacy and conditions such as obesity, diabetes, and hypertension and other health needs.
