Thank you for your interest in Faith in Prevention!  To apply for the program, please follow the steps below:

  1. Complete the online Faith Community Health Assessment survey
  2. Complete the application for mini-grant funds, below.  This requires the following files to be uploaded:
    • A budget (templates are available to download in Word or PDF)
    • Letter of Intent, on organization letterhead
    • A W9 Form, with the first page filled out entirely (For Box 3, if you are a 501(c)3 nonprofit, check the ‘Other’ box and write in ‘501(c)3 nonprofit.’)

As needed, our Outreach Officer can provide you with assistance and resources you may need to complete the application.

Once approved, you and your team will need to attend one Lay Leader Training, where we will go over the curriculum and how the program is facilitated.

Faith in Prevention Mini-Grant Application

  • Faith Community Information

  • Please use the full, official name of the organization, as it appears on tax forms. This is the name we will address mini-grant checks to.
  • Program Plan

  • Please indicate the time you would like the program to start. Keep in mind, we recommend having a 2-hour time slot for each day of the program (example: 12:00 PM - 2:00 PM). We will do our best to accommodate time requests.
  • File Uploads

    Please upload the following required documents as part of your application: -Budget -Letter of Intent -W9 form
  • Please provide a budget for how your faith community plans to spend the $2,100. You may use a template we provided in the instructions at the top to create your budget (in Word, Excel, or PDF), or create your own, detailing your plan for the use of the funds. The majority of funds should be used on implementing policy, systems, and environmental changes (PSE changes) at the faith community that encourages health and wellness for all members or congregants.   • Please ensure it is on organization letterhead.
