Faith in Prevention Mini-Grant Application

  • Faith Community Information

  • Please use the full, official name of the organization, as it appears on tax forms. This is the name we will address installment checks to.
  • Program Plan

  • Please indicate the time you would like the program to start. Keep in mind, we recommend having a 2-hour time slot for each day of the program (example: 12:00 PM - 2:00 PM). We will do our best to accommodate time requests.
  • Budget

Please provide a budget for how your faith community plans to spend the $2,100. You may use a template we provided in the instructions at the top to create your budget (in Word, Excel, or PDF), or create your own, detailing your plan for the use of the funds. The majority of funds should be used on implementing policy, systems, and environmental changes (PSE changes) at the faith community that encourages health and wellness for all members or congregants. For more information about what policy, systems, and environmental changes are, please visit: http://www.cookcountypublichealth.org/files/CPPW/PSE%20Change.pdf.
