Trenton Health Team Selects GoMo Health™ Platform For Underserved Community Pilot Program

Trenton, NJ – Dec. 5, 2013 – Gold Mobile (GoMo Health™), a leading provider of a remote patient engagement system for hospitals, healthcare plans, and community clinics, has today announced that its flagship behavioral communications platform, Concierge Care™, has been chosen to engage members of the Trenton community in a pilot program designed to improve health outcomes. The initial program, funded by The Horizon Foundation for New Jersey grant awarded this September, will focus on two distinct populations: patients with diabetes and those who are high-utilizers of emergency department services.

The Concierge Care platform utilizes behavioral engagement methods, enabling healthcare organizations to personalize their communications throughout the day in order to change lifestyle habits and encourage better health. Concierge Care has a unique way of communicating complex content in small, “snackable” bites, while also empowering a patient to initiate a digital conversation with his or her healthcare professional or social worker.

“The Trenton Health Team has been working hard to encourage healthy living and to improve the quality of life of those most in need,” says Greg Paulson, Deputy Director of the Trenton Health Team. “Incorporating Concierge Care into our pilot program was a clear choice because the platform connects with people in-the-moment, as they are dealing with their health problems, and uses its behavioral approach to empower healthy changes.”

The Horizon Foundation for New Jersey promotes health, well-being, and quality of life in New Jersey’s communities. Priority areas include health and the arts.

GoMo Health has already been deployed in Medicaid and CHIP programs for use in reducing costs, improving remote patient care, and complying with industry and quality measurement standards, such as HEDIS and STAR. The behavioral platform has also been incorporated into health programs to provide a mobile-based Discharge regimen and promote ER avoidance.
