Trenton Health Team Initiatives Featured By Agency For Healthcare Research & Quality In Prestigious Healthcare Innovations Exchange

Policy Innovation Profile highlights THT’s community-wide initiatives to enhance healthcare access for vulnerable populations

TRENTON, NJ-The Trenton Health Team’s (THT’s) innovative efforts to increase healthcare access are featured in this month’s issue of Health Care Innovations Exchange, published online by the Agency for Healthcare Research and Quality (AHRQ). THT’s free clinic for uninsured children and targeted interventions for sickle cell anemia and substance abuse issues were among the community–wide innovations described in the current issue.

“Through our various initiatives, the THT has significantly cut wait times for appointments, enhanced patient-provider continuity, and reduced readmissions and ED use,” said Dr. Ruth Perry, Executive Director of The Trenton Health Team.

In addition to these efforts to improve healthcare for Trenton’s vulnerable populations, THT has developed a health information exchange (HIE) to facilitate data sharing and population health management. THT will also be applying to participate in New Jersey’s Medicaid Accountable Care Organization (ACO) demonstration project.
