The Horizon Foundation for New Jersey Awards Grants to Support Community-Based Health and Arts Programs Throughout the State

The Horizon Foundation for New Jersey announced today the award of $691,250 in grants to 35 non-profit organizations that are working to improve the health and quality of life of New Jersey residents. The Horizon Foundation for New Jersey is the charitable arm of Horizon Blue Cross Blue Shield of New Jersey, and these grants represent the third round of awards in 2013 to community-based organizations in the state.
“The Horizon Foundation for New Jersey is committed to supporting non-profit organizations that are making a positive impact in the lives of New Jersey residents,” said Robert A. Marino, Chairman of the Board of Directors of The Horizon Foundation for New Jersey and Chairman and CEO of Horizon Blue Cross Blue Shield of New Jersey. “We commend these organizations for their work and proudly support their efforts to enrich the quality of life in New Jersey’s communities.”
The third round of awards includes the following 35 grants:
- Algonquin Arts in Manasquan, received a grant of $10,000 to support its Page and Stage Program. The program uses theatre arts to bring story characters to life to captivate and foster an appreciation for reading in young children. The grant funds the program at learning centers serving Head Start children in Asbury Park, Neptune, Howell, Keyport, Red Bank and Freehold.
- American Labor Museum/Botto House National Landmark in Haledon, received a grant of $10,000 to support the Saturday Arts Program. The program provides free visual and performing arts classes to children in grades 3-5 living in Paterson and surrounding towns.
- Appel Farm Arts and Music Center in Elmer, received a general operating support grant of $10,000 to fund its community arts outreach programs.
- The Arc of New Jersey in North Brunswick, received a grant of $25,000 to support the Healthy Lifestyles Project. The grant funds a health literacy curriculum for individuals with disabilities. Participants receive information about adopting a healthy lifestyle, preventative health exams and screenings, managing health concerns and chronic conditions and navigating the health care system.
- Bergen Performing Arts Center in Englewood, received a general operating support grant of $20,000 to fund its 2013-2014 season and arts education programs.
- Connection for Women and Families in Summit, received a grant of $20,000 to support The Pathways Women’s Cancer Teaching Project. The project trains women cancer survivors to become patient educators. The training provides health care professionals, students, and the community atlarge with an enhanced understanding of the perspectives, stresses and needs of women with cancer.
- Diocese of Metuchen in Metuchen, received a grant of $25,000 to support St. John’s Family Health Center’s Diabetes Management Education Program. The program screens patients for diabetes and provides no cost or affordable treatment to those diagnosed with the health condition. The program also offers case management services, nutritional education in English and Spanish, medication and lab assistance, and evaluations by a podiatrist and optometrist.
- The Discovery Orchestra in Warren, received a general operating support grant of $10,000 to fund its 2013-2014 season and educational programs.
- Family & Community Services of Somerset County in Bound Brook, received a grant of $16,000 to support Clinical and Psychiatric Services for Depression. The program provides psychiatric services for economically and disadvantaged clients diagnosed with depression.
- Greater Newark Conservancy in Newark, received a grant of $40,000 to support the Nutrition Education Program. The program teaches better nutrition through education, gardening and access to fresh produce. The grant also supports the construction of the Hawthorne Avenue Urban Farm in Newark.
- Grounds For Sculpture in Hamilton, received a grant of $11,750 to support the Enabling Creativity Program. The grant funds a collaborative art program with Enable Inc., a Princeton-based nonprofit serving individuals with physical and/or mental disabilities and provides participants with monthly hands-on art workshops at GFS during the winter and spring of 2014. The workshop series will culminate in a six-week exhibition of participants’ art work.
- HOPES Community Action Partnership in Hoboken, received a grant of $20,000 to support the HOPES on the Road Program. The program connects individuals residing in low-income neighborhoods in Hudson and Union counties, especially seniors and individuals with limited English proficiency, with information and assistance to locate health care providers and services at accessible locations with assistance from a translator.
- Hunterdon Art Museum in Clinton, received a grant of $21,000 to support the expansion of the Arts in the Schools Program. The program connects middle school students in Hunterdon and Warren counties with artists from the Museum’s current exhibition via Skype on-line chat that is followed by a hands-on project.
- Ironbound Community Corporation in Newark, received a grant of $25,000 to support the Ironbound Children’s Asthma Program. The program offers case management services to parents, as well as information and training on how to treat, identify and prevent risk factors for asthma and respiratory ailments.
- Jazz House Kids in Montclair, received a general operating support grant of $10,000 to fund year-round jazz education and performance programs for students from low-income communities in Northern New Jersey. The students receive music classes and mentoring by professional teaching artists and attend an intensive summer workshop for educational and cultural enrichment.
- Jewish Family Services of Atlantic County in Margate, received a grant of $19,000 to fund the Integrated Health Partnership Program. The program helps individuals with chronic health conditions better manage their health to reduce hospitalizations and hospital re-admissions.
- Jewish Vocational Service of Metro West in East Orange, received a grant of $12,500 to support the JVS Wellness Works Healthcare Initiative. The grant provides health care case management services for patients ages 55 and older with severe disabilities, onsite workshops and an annual health fair for patients and staff.
- Liberty Science Center in Jersey City, received a grant of $25,000 to support the Beyond Rubik’s Cube exhibit, which opens in April 2014 on the Cube’s 40th anniversary. The exhibit fosters critical thinking skills as it examines how the globally-popular Rubik’s Cube connects to a spectrum of fields, including mathematics, engineering, politics, and art.
- McCarter Theatre Company in Princeton, received a grant of $10,000 to support the Access to the Arts Program. The program provides services for barrier-free access, enabling all community members to attend performances at the theatre.
- Mill Hill Child and Family Development Corporation in Trenton, received a grant of $16,000 to support the Springboard Health Literacy Initiative. The initiative supports Trenton’s at-risk children and families in leading healthy lifestyles and managing their health. The initiative helps parents to communicate effectively with health care providers, and equips them with knowledge about family wellness and treatment plans, medical records and appointment organization methods, nutrition education, cooking workshops and mental health education.
- Montclair Art Museum in Montclair, received a grant of $15,000 to fund the Free Family Days Program. The program features guided tours of exhibitions, gallery activities, and live musical, theater and dance performances.
- Montclair Film Festival in Montclair, received a general operating support grant of $20,000 to fund the 3rd Annual Montclair Film Festival. The festival, scheduled for April 29 to May 4, 2014, features over 80 films and offers free screenings, year-round education programs and other events. The grant also funds the 5th Annual Free Summer Series Under the Stars, featuring outdoor film screenings at locations in Montclair and in Essex County from May to September 2014.
- Newark Arts Council in Newark, received a general operating support grant of $15,000 to fund its school based Arts Education Initiative, ArtStart Program, Open Doors Studio Tour and website.
- The Newark Museum in Newark, received a general operating support grant of $30,000 to support public programming at the Horizon Plaza.
- New Jersey Ballet Company in Livingston, received a general operating support grant of $10,000 to fund repertory and educational activities.
- New Jersey Chamber of Commerce Foundation in Trenton, received a grant of $50,000 to fund the New Jersey Action Coalition, which was convened to advance the Future of Nursing: Campaign for Action initiative. The Coalition works to implement the recommendations of the Institute of Medicine’s Future of Nursing report.
- New Jersey Pediatric Council on Research and Education (NJPCORE) in Hamilton, received a grant of $50,000 to support the New Jersey Immunization Network (NJIN) Immunization Initiative. Through this collaborative project, NJPCORE and NJIN will work with select pediatric practices in Ocean and Burlington counties to have their patients, from birth through two years, immunized against all 14 recommended vaccine- preventable diseases. The goal is to establish practices for improving immunization service delivery and immunization rates within each practice. The grant also supports the practice teams’ use of the mandated New Jersey Immunization Information System /Registry to ensure accurate and complete immunization documentation for patients aged seven and under.
- Park Theatre Performing Arts in Union City, received a general operating support grant of $10,000 to fund the Arts Education for Low-income Urban Children Program. The program’s arts and education classes are tailored for the Hispanic students they serve.
- Partnership for a Drug-Free New Jersey in Millburn, received a grant of $15,000 to support the Fifth Grade Parent Alert Initiative. The initiative is offered free of charge to all New Jersey schools and educates parents about the dangers of substance abuse and how to discuss these dangers with their children.
- Passage Theatre Company in Trenton, received a general operating support grant of $10,000 to fund the production of new plays, community programs and The State Street Project, which offers play writing and performance experiences to Trenton’s youth.
- Renaissance Newark Foundation in Newark, received a $10,000 grant for “The Rule,” a film documentary about Newark Abbey and St. Benedict’s Preparatory School. The grant helps underwrite post-production costs, the production of a digital master copy and a companion curriculum guide.
- South Street Theatre Co. (DBA) Mayo Performing Arts Center in Morristown, received a $10,000 grant to support arts education and community outreach programs for talented youth ages 11 through 18.
- State Theatre Regional Arts Center at New Brunswick in New Brunswick, received a $30,000 grant to support the Artist-in-Residence Program.
- Trenton Community Music School in Trenton, received a $10,000 general operating support grant to fund year-round music instruction and performance programs for low-income students in the Trenton area.
- Trenton Health Team in Trenton, received a $50,000 grant to support the Go Mobile for Health Program. The program uses mobile communications to educate diabetic patients on how to work with health care providers to increase their knowledge on self-caring for their disease in order to reduce and prevent use of critical and emergency care. Patients receive appointment and medication reminders, disease information, help in making appropriate health decisions and in navigating the health care system.
