Trenton Health Team works with partners to make a meaningful, practical difference to the health indicators that matter most to Trenton residents.
This page is currently under construction as our team works to built out a way of tracking and reporting on relevant data.
We have selected 16 of these indicators to focus on, and invite accountability for our role in impacting health in the following areas:
Family Health
- Percent of children testing positive for elevated lead levels (2022 = 6.5%)
- Percent of Medicaid members experiencing severe maternal morbidity
- Percent of students who are chronically absent in Trenton-area schools (TPS= 42.2%)
Community Development
- Percent of Trenton households that are housing cost burdened (i.e. that spend 30% or more of their household income on housing)
- Percent of properties that are vacant, abandoned or deteriorated
- Number of registered businesses
Food Security
- Percent of residents that are food insecure
- Percent of residents living in low income neighborhoods with low access
Substance Use
- Age-adjusted drug-induced death rate per 100,000
- Rate of substance-use related emergency room visits.
Health Education, Promotion and Management
- Percent of Trenton adults with unmanaged diabetes
- Percent of Trenton adults with primary care visit in the past year
- Percent of Medicaid members with a hospital encounter in past year for potentially preventable problems
Medicaid Support
- Number of people enrolled in Medicaid
- Percent of Medicaid members who retain coverage
- Percent of Medicaid members with a primary care visit in the past year
